For years, estrogen has been perceived as a hormone to approach with caution, especially concerning breast cancer risk. The narrative often portrayed estrogen as the "big bad wolf" when it came to breast cancer, with many women steering clear of hormone therapy due to fears of increased risk. However, after more than two decades of rigorous research and clinical trials, a ground-breaking revelation has emerged: Estrogen may actually lower the risk of breast cancer.

A recent comprehensive study published in Obstetrics & Gynecology (2024) debunked many of the long-standing myths surrounding estrogen therapy. The study highlights how hormone therapy, particularly using estradiol, has shown not only a significant reduction in menopausal symptoms but also a decrease in breast cancer mortality. According to Dr Barbara Levy and Dr James Simon, the authors of the study, estrogen alone - specifically conjugated equine estrogen - was found to reduce breast cancer mortality by 45% during an 18-year follow-up​.

Shifting Perceptions of Estrogen

The past decades have seen fluctuating enthusiasm regarding hormone therapy. Much of the hesitancy can be traced back to the 2002 Women’s Health Initiative (WHI) study, which raised alarms about the potential risks of hormone therapy. However, it's now understood that the WHI trial, which primarily studied older, asymptomatic women, did not accurately reflect the benefits of hormone therapy for all menopausal women. As Dr Levy and Dr Simon noted, “Our patients deserve a more nuanced, individualized approach” when it comes to managing menopause and hormone therapy.

More nuanced studies, like the 2020 long-term follow-up of the WHI, demonstrate that conjugated equine estrogen alone led to a significant reduction in breast cancer incidence. This has shifted the understanding of estrogen from a hormone to fear to one that may protect against breast cancer in certain formulations.

The confusion surrounding estrogen and breast cancer risk likely stems from the wide array of hormone therapy formulations and delivery methods, as noted in the study. Estradiol, a form of estrogen that is identical to what the body produces, binds to receptors differently than other estrogen types, offering distinct benefits without the same risks.
